Tracking Form	All shipments sent to Kelowna Instruments Ltd, require a completed copy of the Tracking Form included with the shipment. Failure to include the Tracking Form, with your shipment, may result in lost shipments or delayed service.

Shipping Label	Please affix a copy of the Shipping Label to the outside of the box containing your shipment(s). Failure to affix a Shipping Label to your shipment, may result in lost shipments or delayed service.

Get Adobe Reader	Adobe Reader is required to view / print downloaded forms.
